ISBAR sheet
RCSI gives you a patient’s documents and its own two-page ISBAR sheet. You fill the sheet in writing, then, no later than five minutes before the end, ring the doctor using only what you wrote. Here the rows are the RCSI rows. Each is marked against the documents: the numbers, the single parameter, what changed, the background, and the time frame from the INEWS protocol. Where the documents give nothing, blank is the right answer.
