Sepsis screening form

The HSE Sepsis Screening Tool for Adults, September 2025 edition, as printed. RCSI says that from 24 January 2026 this is the only version used at the OSCE. You get a patient, the observations and what you can see at the bedside; the form is filled in step by step in the order it branches on paper, and each tick is marked against the form’s own wording.
